A Russian terrorist organisation known as Anarchy 99 has been able to get their hands on a biological weapon known as “Silent Night,” which has been thought to have been lost since the collapse of the Soviet Union. An undercover operative dispatched by the American National Security Agency to retrieve it was discovered and eliminated without difficulty by Anarchy 99.

NSA Agent Augustus Gibbons advises that someone who does not have ties to the US government be sent, such as Xander “XXX” Cage, who is wanted by the FBI for actions of protesting against a senator. Cage is a professional extreme sports athlete. Cage is taken into custody by Gibbons, and after passing two field tests, he finally agrees to take the job after being informed that the only other option available to him is to go to prison.
